Download the form and mail it to us.

If your copy of Adobe Acrobat Reader does not immediate open the
document after the download is complete, start the Adobe Acrobat
reader software on your computer, and then find and click on the file
that was downloaded onto your computer (Dupform.pdf).
We can now take credit card payments for duplicate awards.
Don't forget to email (or write the info on the form)
to let us know that you paid online.

computer, you can go to the Adobe Website.

If all else fails, email us at info@chrisawards.org and we will mail
you the appropriate form.
