The Division Information,
Guidelines, & Fee Schedule is available as an Adobe
.pdf document of 5 pages. You can download that document here. If your copy
of Adobe Acrobat Reader does not immediately open the document after
the download is complete, start the Adobe Acrobat reader software
on your computer, and then find and click on the file that was downloaded
onto your computer "divdown02d.pdf".
You
do not need to return the Division Information, Guidelines
& Fee Schedule document with your Entry Form (download the Entry

Entry Form for submissions is available in two forms:
(1) An
online page that you can print out and return with your submission.
Size your screen so that everything lines up logically, before you
print. If this is the option you would like, please go to the 2003
ENTRY PAGE.
This will open in a new browser and you will be able to print it
out.
(2) An
Adobe Acrobat (PDF) file. If this is your option, please click here to download the 2003 PDF file (ChrisAwd02.pdf).
This is the Entry Form ONLY and does not include the Division info
or fee schedule. If your copy of Adobe Acrobat

ATTENTION
STUDENTS!
We
need both your home (permanent) address and your school's
address (department). The jpeg image form spells this out, but the
Entry Page doesn't specifically have a space for this. You may use
the Production Company address "block" for your personal
address (permanent).
If all
else fails, call us at (614) 841-1666 (9 am - 5 pm Eastern Standard
Time - note that we are part-time staff), and we will fax or mail
you an entry form.
